Institution Identification - School Names

You have reached the Oregon Department of Education Institutions Database webpage. Here you will find institution identification information for institutions that do business with ODE such as schools, districts, ESDs, and other institutions.

Institution Search

Use this application to lookup institution information from our Institutions Database. You can search by name, location, or identification number to find identification information about schools, programs, school districts, ESDs, and other educational institutions. Also find a .csv file download of Oregon public schools here.

Institution Request Form

This form is specifically for district and school requests. It is used for both directory updates (such as staff changes) and major district or school updates (such as changing a grade range, adding a school, closing a school, or changing the name of a school). Directory updates can be submitted at any time in the year but major changes must be in to ODE by SEPTEMBER 15 for changes to be effective that year. Failure to submit your institution changes in a timely manner can impact reports and your institution’s ability to submit data. If you want to submit a change to your school or district’s information, you can complete the form above and send the changes to the ODE Institutions Database Team.

Requests for 2025-26 will not be processed until after March 1, 2025.

Non District Changes

The Institutions Database is used for all entities doing business with ODE in some capacity, including non-standard public schools (eg: private schools, preschools, nutrition programs, etc.). Use the appropriate online form below to make institution requests based on your specific needs. Requests may be for new institutions or for changes to current institutions. For changes to current institutions, you will need to know your current institution ID which you can find on the Institution Lookup Tool. If you do not know what you need to request, please work with your contact at ODE.


Oregon School Directory

The Oregon School Directory is an online directory listing phone numbers, addresses, and key staff people in all public schools and districts in the state. It further contains information on Oregon Department of Education staff, additional information about private schools, and statistics on the number and composition of Oregon schools.

Public schools, districts, charter schools, and a number of other entities are required to have an ID to do business with ODE. To make changes or request an ID, save the Institution Request Form from this page to your own system, fill out the form, and email it to the ODE Institutions Database Team. Failure to complete the form as directed for each section may delay your request. Many requests require additional supplemental documentation which can be found on page 9 of the Institution Request Form. For non-traditional entities doing business with ODE please refer to information for Non District Changes on this page or contact the ODE Institutions Database Team for guidance.

Temporary Address Changes Due to Emergencies

If a temporary move due to an emergency (building damage due to ice storm, fire, etc.) does not extend from the emergency school year into the next school year (as defined by the district), no notification of address change is required. If the temporary address change will persist into the next school year, notification is required. Notification will require the standard Institution Request Form for an address change and Board Minutes showing board notification of the temporary change.
