Medically Fragile/School Nurse Data Collection
The annual Medically Fragile and School Nurse data collection will be opening on May 5, 2022. This collection asks for information on student health and school nurse staffing. Please review the Medically Fragile Data Collection Definitions and Guidance the powerpoint slides for instructions on how to input data. When you are ready to input data, visit the ODE District Website. Submit your School District's data by June 24, 2022.
Dental Screening Data Collection
The annual Dental Screening data collection will also be opening on May 5, 2022. This collection asks for information on student dental screening certificates. Please review Dental Collection FAQ and the powerpoint slides for instructions on how to input data. When you are ready to input data, visit the ODE District Website. Submit your School District's data by June 24, 2022.
General Reminders:
These Data Collections are to be submitted at the school district level only. Submissions should include all information from public school programs within the school district. School districts must work with ESDs and Charter Schools to ensure that submissions are complete and occur only once.
