Welcome to Annual Transportation Reports

These reports are for the school year ending June 30 each year. All of the following parts of the report are due to the Oregon Department of Education by September 1 each year. To submit the annual reports, submit the Transportation Information data to the consolidated collections (directions below) and submit all other forms using one of the following methods:

• Scan & upload directly to ODE
  
• Scan and email to buslicense@ode.oregon.gov
  (send as an attachment, do not use secure file transfer as no personally identifiable information is in these reports)
  
• Mail reports to:

All Pupil Transportation Entities

• Consolidated Collections, Transportation Information 
  Instructions for submission to the consolidated collections by school districts.
  Private schools, Head Start programs, and other entities that are not K-12 districts should use Form 581-2249-M to submit this information.
  
• Form 581-2256, Annual Vehicle Certification Report (Computer Turnaround Form) 
  This form contains all districts, schools, and programs. Please search for your specific entity when accessing this file.  To be signed by an authorized district administrator.
• Form 581-2256, Annual Vehicle Certification Report Addendum
  This form contains all districts, schools, and programs. Please search for your specific entity when accessing this file. Use to list assets not accounted for in the existing fleet list. 
  
• Form 581-1453, Certification of Drug and Alcohol Testing Compliance
  To be signed by Designated Employer Representative (DER) and District Superintendent (or private program director).
  

School Districts Only

• Form 581-3171, Bus Garage and Depreciation Schedule and Mileage Report
  

  This form contains all districts, schools, and programs. Please search for your specific entity when accessing this file. This form must be signed by the district clerk or business manager.  Numbers will be used to complete calculations for State School Fund reimbursements, and are vital to this payment.

Complete & Retain Locally

• Form 581-2255-M, Annual Vehicle Inspections and Maintenance Report
  Complete and retain copies locally at shop office and superintendent's office (do not include in submission).
