Columbia County- The Board reprimanded the nursing assistant certificate of Adrian Lujano (202004854CNA) for failing to maintain professional boundaries. He resides in Saint Helens, Ore.
Jackson County- The Board placed the registered nurse license of Kari M. McGrew (202006695RN) on probation for 24 months for using intoxicants to the extent injurious to herself or others. She resides in Medford, Ore.
Jefferson County- The Board reprimanded the registered nurse license of Judith M. Anderson (098003196RN) for practicing nursing while impaired and failing to conform to the essential standards of acceptable nursing practice. She resides in Madras, Ore.
Lane County- The Board reprimanded the nursing assistant and medication aide certificates of Mandie R. Ross (000033887CNA/200520081CMA) for failing to accurately document CNA tasks and unauthorized removal of drugs from any setting. She resides in Eugene, Ore.

Multnomah County- The Board placed the registered nurse license of Gabrielle E. Dellabough-Miranda (201907702RN) on probation for 24 months for obtaining unauthorized controlled medications and practicing nursing while impaired. She resides in Portland, Ore.
- The Board suspended the practical nurse license of Celina N. Leeper (200930402LPN) for 30 days, followed by a one-year probation, for entering falsified documentation into a health record, documenting nursing practice implementation that did not occur, and demonstrated incidents of dishonesty. Tillamook County- The Board accepted the voluntary withdrawal of William E. Bursaw's CNA application for conduct unbecoming a CNA in the performance of duties and failing to fully cooperate with the Board during an investigation. He resides in Nehalem, Ore.
