- The Board reprimanded the registered nurse license of Michael A. White (202104416RN) for implementing policies that jeopardize client safety and failing to conform to the essential standards of acceptable nursing practice. He resides in Coos Bay, Ore.
Jackson County
- The Board suspended Michelle A. Green’s licensed practical nurse license (202109822LPN) for 120 days with conditions for documenting nursing practice that did not occur, falsifying data, and failing to conform to the essential standards of acceptable nursing practice. She resides in Medford, Ore.
- The Board reprimanded the registered nurse license of Cindy Park (201702105) for failing to take action to preserve a client’s safety and failing to conform to the essential standards of acceptable nursing practice. She resides in Phoenix, Ore.
Klamath County
- The Board accepted the Voluntary Surrender of Tiffany Fregoso’s registered nurse license (202005781RN) for demonstrated incidents of abusive behavior and failing to report incidents of abuse, neglect, or mistreatment. She resides in Klamath Falls, Ore.
Lake County
- The Board accepted the Voluntary Surrender of Kylie K. Leach’s registered nurse license (201808910RN) obtaining unauthorized medications, failing to maintain boundaries with a client, and failure to report her conviction of a crime within 10 days of the conviction. She resides in Lakeview, Ore.
Lane County
- The Board placed the registered nurse license of William W. Henry (201405532RN) on probation for 24 months for the unauthorized removal of drugs from the workplace, entering inaccurate information into a health record, and documenting nursing practice that did not occur.
