The Board of Nursing meets 12 times a year and takes disciplinary actions during each meeting.  Listed below are the meetings held for the past year.  The status and any disciplinary documents for individual nurses or nursing assistants licensed in Oregon are available on the OSBN Verification System.

  • The Board accepted the Voluntary Surrender of Shannon Chambers's registered nurse license (202008369RN) for practicing nursing while unable due to a mental impairment.  She resides in Grants Pass, Ore.  (NCSBN ID #41148069)

  • The Board accepted the Voluntary Surrender of Aaryo Bakhtiar's registered nurse and certified registered nurse anesthetist licenses (201404127RN / 202000790CRNA) for violating the terms and conditions of a Board Order.  He resides in Lake Oswego, Ore.
  • The Board reprimanded the practical nurse license of Elizabeth N. Crum (201911197LPN) for falsifying data and documenting nursing practice that didn't occur.  She resides in Lake Oswego, Ore.
Marion County

  • The Board reprimanded the registered nurse license of Geanna M. Berrier (200942111RN) for falsifying data and documenting nursing practice that didn't occur.  She resides in North Bend, Ore.
  • The Board reinstated the registered nurse license of Bobbie M. Felton (200942261RN) and placed it on probation for 12 months for violating the terms and conditions of a previous Board Order.  She resides in Coquille, Ore.
Deschutes County
  • The Board accepted the Voluntary Surrender of Sue A.   • The Board suspended the registered nurse license of Jon E. Myrold (202002915RN) for 30 days for demonstrated incidents of reckless behavior and failing to administer medications in a lawful manner.  He resides in Bend, Ore.
Jackson County
  • The Board placed the nurse practitioner license of Lauren E. Hodgdon (201809297NP-PP) on probation for 12 months for prescribing medications to herself, entering inaccurate documentation into a health record, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Talent, Ore.
Jefferson County
