Case Management System Goals

Creating better outcomes

The Case Management System will help all people in Oregon receiving intellectual and developmental disabilities services. The goal is to be open, efficient and improve data collection and reporting.

The new system will make processes simpler, saving time that case managers spend on document handling. This means they can spend more time helping people.

For example, case managers won’t have to manually track assessment and service plan due dates for each person. Instead, the system will automate tracking and notices.

The system will have data collection and reporting on race, ethnicity, language, disability, sexual orientation and gender identity. This data will highlight underserved communities. This will make services and outcomes accessible and fair for everyone.

The system will connect with statewide software, helping reporting capacity. ODDS and CMEs can access and view data from it to guide improvement efforts.

For instance, a report may show low use of a service in a particular area. We can look at the reasons and create strategies to improve access to that service.

Individuals receiving services will have the choice to use an online portal to view their info. This will give them another way to shape how they receive services to achieve their goals.

The new system will work on mobile devices such as phones and tablets. With this, case managers can use case management tools and access data while in the field.

Its software will meet changing needs and work with other state systems. This makes the process easy for users and reduces repeat data entry across systems.
