Requesting Results or Bloodspot Specimens

  • Ask for Lab Results (includes sickle cell results) - Please visit the webpage for Requesting Test Results for instructions.
    • Records are not available for patients older than 6 years of age. Newborn bloodspot screening results are kept for 6 years and then are securely destroyed. For patients requesting sickle cell test results for college athletics, the laboratory is unable to provide these results.
  • Ask for the Bloodspot Specimen - Please visit the webpage for Residual Bloodspot Retention, Use, and Release for instructions.

Exception

  • Ask to Opt Out of Screening - If you adhere to a religion that opposes this testing, you may apply for a religious exemption using the Religious Exemption Form (pdf). If you do not send this form to the laboratory, your baby must be tested.
