DEQ’s UIC program is using a new electronic data management system called Your DEQ Online, you can set up your account and start submitting notifications and pay fees electronically. As of that date, all notification and application forms will be on Your DEQ Online and will not be available in PDF or Word format.
Applications for authorization by rule
The following forms are used to apply for authorization of a UIC by rule. A UIC must meet certain
conditions to be authorized by rule. DEQ processes applications for authorization by rule within two weeks of receiving a completed application. The following forms are now available in Your DEQ Online. For more information please visit Your DEQ Online Help page.
- Stormwater Discharges - roof only
- Stormwater Discharges - all surfaces
- Non-Stormwater - aquifer storage and recovery, low-temperature geothermal, remediation, and other
Applications for authorization by permit
The following forms are used to apply for authorization of a UIC by permit. UIC owners apply for authorization by permit if UICs do not meet the conditions for authorization by rule, or if authorization by rule is cost-prohibitive. - Individual Permit (WPCF-N) - Class V Stormwater, Class II Oil & Natural Gas, and Class V Geothermal
- General Permit (1900B) - Class V Geothermal
- General Permit (1200U) - Class V Stormwater

- Name Change and/or Transfer of Registration and Authorization
