Registering a Complaint with the Board


The Oregon Board of Pharmacy responds to all complaints received. If you believe a violation of Oregon Administrative Rules or Oregon Revised Statutes might have occurred, you may want to file a complaint with the board. Complaints can be made by completing and submitting the online complaint form located below.
