Pharmacy Drug Outlets - Retail & Institutional Applications
Be sure to submit a complete application to avoid delays in registration. All applications submitted to the Board that are not complete and processed within 6 months from applicant signature will be expired. Once expired, applicants who wish to continue with the application process must reapply by submitting a new application, along with all documentation, and all fees.

All out-of-state Drug Outlets, Drug Distribution Agents, Manufacturers and Wholesalers must include a copy of the resident state license, as well as license verification for that license with their application. This is required for all new outlet applications, location changes and name changes.

In addition to submitting an Oregon application and fee, all out-of-state Retail Drug Outlets, Drug Distribution Agents, Manufacturers and Wholesalers must include a copy of their active license and license verification from their resident state. Nonprescription Drug Outlet Information & Applications
Nonprescription Drug Outlet - Class A is any outlet that sells over-the-counter nonprescription medication directly to the consumer.
This includes stores, gas stations, hotels, gift shops and taverns as well as out of state businesses offering over-the-counter medications for sale to Oregon residents through catalog or internet sales.
If an outlet is selling and/or distributing over-the-counter medications to any other outlet and is also selling directly to the consumer, two registrations are required; a Wholesaler Class II Registration and a Nonprescription Drug Outlet Registration. This includes catalog and internet sales.
Nonprescription Drug Outlet - Class C (Medical Device, Equipment & Gas Drug Outlet) is any outlet that sells medical devices and/or medicinal gasses and over-the-counter medications to a consumer.
If an outlet is selling and/or distributing medical devices and/or medicinal gasses and over-the-counter medications, to any other outlet and is also selling directly to the consumer, two or more registrations are required; Wholesaler Registration(s) and a MDEG Outlet Registration. This includes catalog and internet sales.
A separate registration is required for each location. This registration does not authorize an outlet to sell any prescription medications. Nonprescription Drug Outlets are prohibited from purchasing or selling any product containing any amount of ephedrine, pseudoephedrine, or phenylpropanolamine.
OAR 855-035 lists the regulations regarding the operation of Nonprescription Drug Outlets.
