County Based Service Model

The county based model of case management services is provided by local health departments in Hood River and Deschutes counties.

The following Standards of Service and forms are used by all county based case management service providers and are required by contract.

Regional Based Service Model

The regional based model of case management is provided by community based service organizations HIV Alliance and Eastern Oregon Center for Independent Living (EOCIL).
