ALERT IIS Enrollment and Training

All schools and children’s facilities in Oregon are required to track immunizations. Immunizations are required for attendance at all elementary, middle, and high schools in Oregon, whether public, private, charter or alternative schools. Immunizations are also required for attendance at all children’s facilities, including preschool, Head Start, and certified childcare programs. Parents should provide a completed Certificate of Immunization Status for each child. Schools and children’s facilities that are authorized will have the ability to print a Certificate of Immunization Status form with immunization dates from the ALERT Immunization Information System if the dates are in ALERT IIS.


Enrollment

In order to access and use the ALERT IIS system, schools and child care programs must first register. To enroll in ALERT IIS as a school or child care program, complete the following:

  1. Complete the Schools/Child Care Facility Site Agreement form.
  2. Read the Confidentiality Policy (every ALERT user must read this document).
  3. Each individual user who will be utilizing ALERT IIS must read and sign the School Individual User Agreement, watch a training video, and print the certificate at the end of the video.
  4. Send the above forms and the training certificate to the ALERT IIS Help Desk.

Training

Live Zoom and Recorded Webinars are designed for school staff and staff that work in child care centers, preschools and head start programs. The topics covered include how to login and search for a child's record, how to create lists of students and reports.
