Reports

  • Children's health complexity data - CCO, county and statewide reports for 2019-2023.
  • Transformation Plans - CCO Transformation Plans for 2013-2015 and 2015-2017. In 2018, the Transformation Plan was replaced by the the Transformation and Quality Strategy.
  • Senate Bill 436 Report (2014) - Effective July 1, 2013, Senate Bill 436 highlighted issues related to children’s health care and established guidelines for coordinated care organizations to focus on children’s health, to the extent possible, in their community health improvement plans. This report assessed CCOs' progress.

  • Transformation in Action - The 2019 Transformation in Action newsletter featured stories about coordinated care organizations’ innovative work, highlighted evidence-based best practices, and celebrated successes.
