Mission: The Oregon Health Authority (OHA) Transformation Center is the hub of innovation and quality improvement for Oregon’s health system transformation efforts to achieve better health, better care and lower costs for all.
Goals: The Transformation Center identifies, strategically supports and shares innovation at the system, community and practice levels. Through collaboration, we promote initiatives to advance the coordinated care model.
The center continues to adopt new strategies to further advance primary care, value-based payment, population health, behavioral health integration, oral health integration, incentive measure work and other cross-cutting supports.

Innovative payment strategy
CCOs have extensive requirements to transition from fee-for-service payment to value-based payment (VBP) models. The Transformation Center manages the following initiatives and technical assistance to support payers and practices in containing cost growth and advancing payment reform.
CCO VBP Roadmap
The
VBP Roadmap's goal is to ensure at least 70% of CCO payments to providers are in the form of a VBP by 2024. The roadmap also includes VBP models in key care delivery areas, infrastructure payments for Patient-Centered Primary Care Homes (PCPCHs), and strategies to promote equity in VBP design.
Primary Care Payment Reform Collaborative
As initiated through Senate Bill 231, the Transformation Center coordinates a
Primary Care Payment Reform Collaborative. This collaborative convenes payers to share best practices on primary care value-based payment methods and aligning initiatives.
VBP Compact
The Transformation Center staffs the
Oregon VBP Compact Work Group, which is a collaborative partnership to advance VBP adoption across the state.

Population health
Community advisory council (CAC) supports
The Transformation Center provides targeted supports and learning opportunities for
CCO community advisory councils (CACs).
Community health assessments (CHAs) and improvement plans (CHPs)
The Transformation Center
collects and analyzes CCOs' CHPs and develops resources for CCOs and their collaborative CHA/CHP partners.
Medicaid Advisory Committee
The Transformation Center staffs the
Medicaid Advisory Committee, which advises OHA, the Oregon Department of Human Services, the Oregon Health Policy Board and the Oregon State Legislature on Oregon Health Plan operations and policies from a consumer and community perspective.
CCO spending programs
Health-related services
Health-related services (HRS) are non-covered services that are offered as a supplement to covered Medicaid benefits to improve care delivery and overall member and community health and well-being. The Transformation Center develops resources and offers technical assistance to guide CCOs through the HRS process.
Supporting Health for All through REinvestment: the SHARE Initiative
The Transformation Center collects and reviews SHARE spending plans and develops SHARE Initiative guidance for CCOs.
Oral health integration
Oregon's health system transformation efforts have focused on preventive care, treating the whole person and care coordination among providers, including oral health providers.
System transformation and quality improvement
CCO incentive metric support
The Oregon Health Authority uses incentive measures to set goals and reward CCOs for improving health care quality and health outcomes of Medicaid members. The Transformation Center provides technical assistance to CCOs and clinics on select measures. For more details, see the CCO Incentive Metrics TA webpage.
Transformation and quality strategies
The Transformation Center collects and analyzes CCO Transformation and Quality Strategies. The center also provides Transformation and Quality Strategy technical assistance.
