Complex Care Collaborative

The Complex Care Collaborative was active from 2013-15. The collaborative was set up for Oregon CCOs, clinical practice leaders and staff working to improve health outcomes for Oregonians who require complex care. Resources and materials from previous collaborative events are listed below:

Collaborative Objectives

  • Improve health outcomes for Oregon Health Plan members who require complex care
  • Support the spread of innovative complex care models throughout Oregon
  • Promote information sharing and networking
