DWS Operator Certification Renewal

Renewal notices were mailed out the first week of October 

If you didn’t receive your notice, you can download a renewal form below, or contact the program: dws.opcert@odhsoha.oregon.gov or 971-673-0321 

When you pay online, make sure you email, fax or mail in your renewal paperwork (see below).

Individual renewal forms are mailed to all currently certified Distribution and Treatment operators in October of every year. If you didn’t receive your notice, download a renewal form below, or contact the program: dws.opcert@odhsoha.oregon.gov or 971-673-0321

It is your responsibility to provide a current mailing address and other contact information and to report any changes on your certification, such as change of mailing address, phone number(s), email, or employer. Update your contact information by emailing the program dws.opcert@odhsoha.oregon.gov or 971-673-0321

Note: If you are a new certified operator this year, you do not need to complete the CEU Self-Certification Affidavit.
