Area Plans

Area Plans address the needs of older adults, adults with disabilities and their caregivers while assessing the strengths and weaknesses of current community resources available to them.

2021-2025 Area Plans, budgets and staffing

Area Agencies on Aging (AAAs) are required to complete Area Plans every four years. The plans are developed with public input and approved by the Oregon Department of Human Services.

Oregon Community Health Needs Assessments and Improvement Plans

Oregon's Office of Rural Health in partnership with the Oregon Health Authority's Transformation Center and Public Health Division, created an interactive map of the most recent Community Health Needs Assessment and Community Health Improvement Plans for Oregon's non-profit hospitals, local public health authorities and CCOs.

These tools may be helpful in making decisions about where to target outreach efforts and creating culturally sensitive and linguistically appropriate outreach and application assistance.
