• Agricultural Amendment Product Registration (APR) Form
  An APR Form is required for the registration of agricultural amendment products.
• Agricultural Mineral Product Registration (MPR) Form
  An MPR Form is required for the registration of agricultural mineral products.
• Fertilizer Manufacturer / Bulk Distributor (FMBD) Form
  A manufacturer-bulk distributor license (type 96) is needed by each out-of-state or in-state business that distributes fertilizer, ag amendment, ag mineral or lime products in bulk in Oregon, or each in-state business that manufactures fertilizer, ag amendment, ag mineral or lime products.
• Fertilizer Product Registration (FPR) Form
  An FPR Form is required for the registration of fertilizer products.
• Lime Product Registration (LPR) Form
  An LPR Form is required for the registration of lime products. A product is considered a lime in Oregon if its calcium and magnesium compounds are capable of neutralizing soil acidity.
