90-Day Nurse Licensure Exception Form

Employers Needing 90-Day Exceptions for Temporary Staffing Shortages:  Employers Needing 90-Day Exceptions for Temporary Staffing Shortages: Oregon law (ORS 678.031(7)) allows licensure exceptions for non-resident nurses in a single staffing assignment. This exception does not apply to nursing assistants. To be eligible for this exception, nurses must have an endorsement or reactivation application on file with the Board prior to employment. Following the initial 90-day exception, the employer may request an additional 90 days.

Requests must be submitted by the entity requesting the exception, not the nurse being hired.

Nurses must remain licensed in another state for the duration of their employment to be eligible for the exception.
