The Health Licensing Office is located at 1430 Tandem Ave. NE, Suite 180, Salem, OR 97301. If you input the address without the suite number into a map program or GPS, it should locate us and send you this direction.

From the North

Take I-5 South. Take exit 260A toward Salem Parkway. From Salem Parkway, turn left on Cherry Avenue NE, then the first left onto Van Ness Avenue NE, then the first left onto Del Webb Avenue NE. Follow Del Webb around till it turns into Tandem Avenue, then look for the Oregon Health Authority/Health Licensing Office sign on the left. Parking is free and plentiful.
