Early Head Start and Healthy Families Forms:

Key Dates for Data Reporting and Quality Assurance

  • Quarterly Quality Assurance Checks
    • These check for missing referrals, forms, case visits, enrollment surveys, exit surveys, etc.
      • Q1 - Early January - End of January
      • Q2 - Mid March - Mid April
      • Q3 - Mid June - Mid July
      • Q4 - Early September - End of September
  • Mid-Year End Reporting and Client Follow-up Data Check 
    • These reports are broken into two sections a quality assurance check (see Q2 above) then a client list check to improve services related to MIECHV performance measures.
      • Timeline: Mid-March – End of June
  • Annual Reporting and Client Follow-up Data Check 
    • These reports are broken into two sections a quality assurance check (see Q4 above) then a client list check to improve services related to MIECHV performance measures.
      • Timeline: Early September - End of December

Data Collection Schedule Tool:

  • EHS/HFA Data Collection Schedule 
    The Data Collection Schedule Tool is available to support home visitors in completing client data forms on-time

    The data collection schedule tells you when each data form is due for a client. It is also a tool to track which forms have been completed and which have not. Be sure to create an individual data collection schedule for each parent or family you enroll.

    Please note: The schedules look best when printed as a PDF rather than an Excel document.

​To report MIECHV data, home visitors must enter data into THEO.  

Continuous Quality Improvement (CQI) is a strength-based process that relies on teamwork to improve processes, services and outcomes. It is an ongoing cycle of collecting data and then testing, implementing, learning from, and revising solutions. 

Continuous Quality Improvement is a required component of the MIECHV grant. Local Implementing Agencies conduct CQI activities as part of working to improve the quality of home visiting services in Oregon.

2023-2025 CQI Learning Collaborative 
CQI efforts will be on improving overall Screening and Referrals practices and processes, including improving timely ASQ-3 developmental screenings as a primary CQI collective change to maximize the efforts gained from the CQI Practicum and spread to LIAs across all models. 
Our overall statewide aim is to increase the percent of children enrolled in MIECHV home visiting programs with timely completion of ASQ-3 developmental screening from 55.7% in FY23 to 60-65% for FY24
