Admission and Discharge Data Collection for Behavioral Health Clients at Acute Care Hospitals

The ACR system is designed to:

  • Facilitate data collection of addictions and mental health treatment data for acute care clients, and
  • Support all publicly funded behavioral health entities and facilities in Oregon who serve acute care clients.

Data Collected

To allow the Oregon Health Authority to report accurate admission and discharge data for behavioral health clients, ACR collects client status data and non-Medicaid service data for events during the client's treatment episode. Providers must submit the following data within 24 hours of admission or discharge as required by Oregon Administrative Rules:

  • DSM diagnosis for adults (age 18 or older) 
  • Admission date 
  • Discharge date 
  • Commitment date 
  • Case number for civil commitments 
  • Legal status 
  • Medicaid eligibility 
  • Oregon Medicaid ID number 
  • Patient demographics (REALD and SOGI) 
  • Warm handoffs
