CCO Community Health Assessment (CHA) and Community Health Improvement Plan (CHP) Reports

CCO submissions

CCOs are required, per Oregon Revised Statue and CCO contract, to complete a CHP, based on a CHA, at least every five years. CCOs are also required to submit annual CHP progress reports to OHA. Annual reporting templates are available on the Guidance and Training webpage

At a glance: tracking community health priorities and progress

  • This CHP Metrics Tracker provides an overview of CHP metrics from 2019-2024 CHP data, including progress, health priorities, and yearly trends. It helps in understanding the focus areas and effectiveness of health initiatives across different service areas by CCO region. **CHP Metrics Tracker is currently under maintenance, please check back at a later date for updated information.

Community health priority matrix

The OHA Transformation Center, in collaboration with the OHA Public Health Division, developed a matrix of community health priorities by agency and region. This tool supports community collaboration with CHA/CHP partners, based on community health improvement plan priority areas. It is not meant to reflect CCO compliance with contractual requirements.

The matrix only includes community health priorities for community health improvement plans that were completed by March 2021 and will be updated at least annually. The matrix does not include priority areas from other CCO, LPHA or hospital plans. If your CCO, LPHA or hospital notes an error in the priorities within the matrix, please contact the Transformation Center
