REALD & SOGI - Providers and Partners

ORS 413.161 requires most health care providers and insurers to collect and report REALD and SOGI information once annually (every 365) for all patients/members. The REALD and SOGI data standards have been codified in the OARs (Chapter 950, Division 30).

Collect Data with Templates and Tools

Asking REALD and SOGI Questions:
All questions must be asked. Patients may select "Don't know" or "Don't want to answer" for any question. Providers may also choose to adapt their own systems and forms to incorporate the REALD and SOGI questions into their existing data collection processes. If doing so:
  • Do not change or remove wording in the questions or in the response choices for each question.
  • Changing the question order is acceptable.
