Application Information and Resources: PPMP

In PPMP laboratories the PPMP tests may only be performed by a Physician, Dentist, Podiatrist or Physician Assistant licensed by the Board of Medical Examiners for the State of Oregon; or a Nurse Practitioner licensed and certified by the Oregon State Board of Nursing. When PPMP tests are performed by other personnel, the laboratory becomes moderate complexity.

A laboratory that performs any test not in the waived or PPMP category will be considered either moderate or high complexity and must meet applicable requirements.

Please don’t send hard copies following a fax or email of a lab form. Double forms can cause duplication and extra work. You can verify receipt of your forms by calling 503-693-4125 or by e-mailing LC.info@odhsoha.oregon.gov.

• Labs NOT located in Oregon: Find your state agency and mail forms to them (pdf)
• Information regarding submission of application (pdf)
• You can now pay your CLIA Fees online
• Pay CLIA fees thru mail (pdf)
• CLIA application form (CMS-116) (pdf)
• Director Qualification Appraisal form (pdf) - This is a required form, please attach completed form to your application.
