General Information
Effective July 1, 2023, all temporary staffing agencies currently operating in Oregon, and those seeking to provide services in Oregon, must apply for licensure. The Health Licensing Office will begin accepting applications on June 19, 2023.
"Temporary staffing agency" means an entity that operates in Oregon for the purpose of providing temporary work to personnel providing health care services or assistance with activities of daily living for or on behalf of client entities.
Application Information
The application process requires the submission of two different forms. Once completed and compiled, mail them together to the Health Licensing Office address provided on the form.
Temporary Staffing Agency Program License Application - Submit one application for each physical location of a temporary staffing agency. Application requirements and fees can be found on the application form.
Temporary Staffing Agency Program - Officer, Director, Ownership and Partnership Form - Submit one form for each officer, director, or persons with ownership, partnership or similar interest in a temporary staffing agency (must submit separate form for each individual). Each individual must also submit to a fingerprint-based nationwide criminal records check. Form requirements and fees can be found on the form.
