Oregon Antimicrobial Stewardship Honor Roll

Program Overview

The Oregon Health Authority Healthcare-Associated Infections (HAI) Program is pleased to present Oregon’s Antimicrobial Stewardship (AMS) Honor Roll. The honor roll was developed to:

  • Recognize acute care and critical access hospitals meeting each of the CDC's seven core elements and working toward meeting priority core elements of antimicrobial stewardship.
  • Promote appropriate use of antimicrobials.
  • Prevent emergence of antimicrobial resistance.
  • Showcase the work of antimicrobial stewards across Oregon.

Benefits

  • Bronze, silver and gold level recipients will receive a certificate and an Honor Roll logo that can be displayed on the facility website and other publications.
  • Recognition on the Oregon AWARE website and HAI newsletter.
  • Demonstration to the public that your facility meets and exceeds national AMS standards.
  • Align your facility’s stewardship program with evidence-based best practices.

Who Should Apply?

  • All acute care and critical access hospitals are encouraged to apply.

Application Process

  • Review criteria in the table below to determine the appropriate recognition level for your hospital; Bronze, Silver, or Gold.
  • Complete and submit the application form. You will be asked to provide examples of your stewardship efforts and attach the following documentation:
    • Statement of commitment from leadership
    • A copy of the facility's antimicrobial stewardship policy
    • A copy of the facility antibiogram (optional)
  • Your answers and document uploads will be saved automatically. If you need to complete your application at a later date, click “Save & Return Later” when exiting the application and follow the prompts. If multiple people will be filling out the application, please share the link to login.

Recognition Levels

  • Facilities will be recognized with bronze, silver or gold status.
  • The application includes basic, intermediate, and advanced criteria for each of the CDC’s core elements and priority core elements of hospital antimicrobial stewardship.

Following are the criterion for the honor roll.
Applicant Criteria (pdf)

  • Le​​adership ​Commitment:​Current Statement of antimicrobial stewardship (AMS) commitment from hospital Leadership​​.
  • Accountability: AMS program/committee has dedicated leader(s).​​
  • ​​​ Pharmacy Stewardship Expertise: AMS program/committee includes a pharmacist.
  • Action:​ Facility specific treatment recommendations for at least 2 common clinical conditions. ​
  • Tracking:​ Monitor antimicrobial use, the impact of interventions or other outcomes (i.e., C. diff infections or resistance patterns)
  • Reporting: Local antibiogram disseminated/available to prescribing staff.
  • Education:​​​ ​Provide annual AMS education to prescribing staff.

  • Leadership C​ommitmen​​t: ​AMS program leaders have stewar​​dship responsibilities in their job description or performance review.​​​​
  • Accountability: ​Multidisciplinary AMS committee that meets to discuss stewardship at least quarterly. ​
  • Action: Preauthorization or formulary restriction.
  • Action: ​Prospective audit with feedback.
  • Tracking:​ Use a hospital-specific antibiogram created within the last 2 years.
  • Reporting: Antimicrobial use data is shared with hospital staff at least annually (through committees, publications, dashboards, etc.)
  • Education:​​​ ​Patient and family antimicrobial education integrated into discharge process.​​

  • Leadership Commitment: Hospital leadership provides dedicated time to​​ manage and conduct stewardship interventions and resour​ces (e.g., IT support, training) to effectively operate the program.​​
  • ​​Accountability: Stewardship Program co-led by a physician and a pharmacist.
  • Pharmacy/Stewardship Expertise:​​ AMS physician and/or pharmacist leaders have completed infectious disease specialty training or certificate program.
  • Action: Antimicrobial use data is utilized to make an improvement in antibiotic stewardship efforts.
  • Tracking: Hospital has submitted at least 9 months of antibiotic use data to NHSN in year prior to application.
  • Reporting: Provider level, unit specific or syndrome specific antimicrobial use reports are provided at least annually.
  • Education:​​​ Ongoing AMS collaboration beyond your facility (e.g., ORASN, UW CSiM membership, mentoring other facilities).



After Application Submittal

  • You will automatically receive an email confirmation with a copy of your application for your records.
  • We will contact you if we have questions or need more information.
  • Notification of designation results will be sent out within 1 month of application.

Renewal Process

  • All recognized hospitals will need to re-apply to maintain their status every two years from their initial award date.
  • Hospitals may choose to apply for an upgrade in their recognition level at any point. The same application form can be used.
