Amendment Evidence Documents

Amendment order processing times vary depending on amendment type and age of record. Click here for current estimations.

If you submitted a request and processing fee, the date your check or money order is cashed represents the verification of receipt. It also helps you estimate the time remaining before your request is processed.

  • Requests are reviewed and processed in the order they were received. If there are any issues with your request, you will be contacted at that time.

Note: Other documents will be accepted as evidence if they meet the requirements. Evidence documents will be returned to the applicant after the amendment is completed.

To add missing names or correct the spelling of a child’s name on their own birth record

Requirements: For a child age 1-6, document must be dated and at least one year old. For a child age 7 or older, document must be dated and at least 5 years old.

Suggestions for evidence document:

  • Original hospital memento birth record
  • Medical record for child with letter from medical attendant/doctor
  • Original immunization record
  • Original baptismal record or religious blessing record
  • Original insurance record/copy of insurance policy application from insurance company
  • Original certified school attendance record or school enrollment record

To add missing names or correct the spelling of an adult's name on their own birth record

Requirements: Document must be dated and at least 5 years old.

Suggestions for evidence document:

  • Original certified school record
  • Original certified child’s record listing registrant as a parent
  • Original marriage certificate or application for a registered marriage certificate
  • Original military discharge record
  • Original, certified DMV driving record
  • Original passport, issued more than five years ago
  • Original naturalization paperwork
  • Certified court documents—divorce/child custody, parenting, etc.
  • Bank/savings account application with certification letter from bank/agency
  • Voter registration document with original envelop from agency
  • Certified copy of application for Social Security Number

To correct the spelling of a parent name, or correct a parent’s date of birth or place of birth

Requirements: Document must be dated prior to the date of birth of the child whose record is being corrected. Changing multiple items may require a court order if they could change the identity of the parent.

Suggestions for evidence document:

  • Original birth record of the parent
  • Original marriage record of the parent
  • Older brother or sister’s birth record with correct information for the parent

To correct typographical error in date of birth of an adult after the medical record of birth is no longer available

Requirements: Document must be dated prior to the 7th birthday of the adult.

Suggestions for evidence document:

  • Original hospital birth record
  • Original baptismal or religious blessing record
  • Certified grade school enrollment record
  • Original insurance policy
  • Certified copy of application for Social Security Number

To correct typographical error in sex at birth of an adult after the medical record of birth is no longer available

Requirements: Document must list the adult registrant in role of mother or father or state sex of adult in the document.

Suggestions for evidence document:

  • Original marriage record
  • Original birth record of a child where the person is listed as the mother or father
  • Medical record that includes sex of person with letter from medical attendant/doctor
  • Original health insurance policy – if sex of person is stated
  • Certified school attendance or school enrollment record – if sex of person is stated
  • Original baptismal or religious blessing record if sex of child is stated
