Correcting/Changing Birth Record Information - Date of Birth, Sex, Time of Birth, Parent Names & Parent Data



In-person customer assistance to change vital records is not available.



Estimated amendment order processing times are currently averaging (5) months. 

  • If you submitted a request and processing fee, the date your check or money order is cashed represents the verification of receipt. It also helps you estimate the time remaining before your request is processed. 
  • Requests are reviewed and processed in the order they were received. If there are any issues with your request, you will be contacted at that time.

Submit the following with each request

  1. Birth certificate order form with full name of child/registrant listed with amended names. Include signature of applicant, photocopy of applicant’s ID, and fees for records.
    For ordering short form certificates, or for births occurring after 2008, please use this Birth record order form (PDF).
    For ordering full image/long form certificates for a birth occurring prior to 2008, please use this Full image birth record order form (PDF).
    OR
    Provide an order number or document showing certificate fees have already been paid and the order is uncompleted;
  2. Original notarized correction affidavit (PDF), if required per the chart below.
  3. $35 amendment fee, if required per the chart below.
  4. Original evidence document, if required per the chart below.

Typographical errors by birthing facility to any item on record

Parents: Submit copy of birth record with errors identified to ensure accuracy of amendment process.

Examples: Spelling of parent’s name, parent’s date of birth, parent’s place of birth, mother’s address, etc.

Birthing facility will submit correction to Vital Records.

Vital Records will contact birthing facility. Birthing facility can correct keying errors but cannot make changes from data provided by parents at time of birth.

Child up to age 21 or until medical record is no longer available

Correct typographical errors in:

  • Child's date of birth
  • Child's time of birth
  • Child's sex at birth

Birthing facility will submit correction to Vital Records.

Vital Records will contact birthing facility. Birthing facility can correct errors based on medical record.

No amendment fee

  • Adult's date of birth
  • Adult's time of birth

Notarized Birth Correction Affidavitsignature of registrant (person named on record) required.

Dated, original evidence document dated prior to registrant’s 7th birthday to correct date of birth.

Correct typographical errors in:

  • Adult's sex

Notarized Birth Correction Affidavitsignature of registrant (person named on record) required.

Dated, original evidence document at least five years old that shows name of person, date of birth or age, and sex required.

Child age 1-17 or under age 1 if hospital cannot correct data based on birth worksheet

Corrections allowed:

  • Correct misspelling of parent name (up to 3 letters)
  • Add missing part of parent name
  • Add second part of parent surname
  • Add or correct parent date of birth, age, or birthplace
  • Reverse parent’s first and middle names
  • Reverse order of parent’s double last names
  • Add or remove hyphen in two-part parent surname
  • Add or remove suffix (Jr., II, III, etc.) in parent name

Notarized Birth Correction Affidavit signature of one parent listed on record required.

Original evidence document dated prior to the birth of the child whose record is being corrected is required.

$35 amendment fee

Corrections allowed:

  • Correct misspelling of parent name (up to 3 letters)
  • Add missing part of parent name
  • Add second part of parent surname
  • Add or correct parent date of birth, age, or birthplace
  • Reverse parent’s first and middle names
  • Reverse order of parent’s double last names
  • Add or remove hyphen in two-part parent surname
  • Add or remove suffix (Jr., II, III, etc.) in parent name

Notarized Birth Correction Affidavitsignature of registrant (person named on record) required.

Original evidence document dated prior to the birth of the registrant whose record is being corrected.

Change a parent's name with a court order of name change. See the Court Order Name Change page for more details.

Change other items with a court order when original amendment request was denied.

Original certified copy of court order and written request of parent if registrant is child under age 18. If registrant is over the age of 18, a written request signed by the person named on the record.
