Social emotional health metric data and reports

CCOs use reach metric data reports created by OHA to inform their work with partners on system-level social emotional health improvement efforts. See statewide and CCO-specific reach metric data reports below.

As part of this incentive metric work, CCOs submit an asset map of existing social-emotional health services and resources in their community. In 2023, most CCOs have two asset maps because CCOs were required to update last year’s specialty behavioral health asset map and complete a new asset map for integrated behavioral health/Patient-centered Primary Care Homes. In some cases these two asset maps are combined into one file.
