OECR Discrimination Complaints

At ODOT, we use the word concern for civil rights related comments, questions, concerns, and requests (CQCRs), and we use the word complaint for civil rights related complaints and grievances. A complaint must be filed with ODOT within 180 calendar days from the date of the issue or incident.

The best way to file a discrimination complaint with ODOT is to use ODOT's online discrimination complaint form.


To file a discrimination complaint without submitting online, print the online form and complete it on paper, or provide the information requested on the form in a separate document. Submit the complaint to ODOT Office of Equity and Civil Rights in one of the following ways:

  • Email the complaint to our Title VI Officer.
  • Fax the complaint to 503-986-6382. Faxes are not monitored daily, so processing delays may apply.
  • Mail the complaint to ODOT Office of Equity and Civil Rights, ATTN: Title VI Program Manager, 800 Airport Rd SE, Salem, OR 97301. U.S. Mail transit times can vary, so processing delays may apply.
