• Commercial Pesticide Applicator (CPA) application - editable PDF
  This form is required for the Commercial Pesticide Applicator license. In addition, applicants must take and pass certification exams. This form requires a signature. Electronic signatures will not be accepted.
• Commercial Pesticide Operator (CPO) application - editable PDF
  This form is required when submitting an application for the commercial pesticide operator license. Also required: Insurance verification and a valid commercial pesticide applicator licensed and employed by the Operator. This form requires a signature. Electronic signatures will not be accepted.
• Immediately Supervised Trainee (IST) application - editable PDF
  This form is required to obtain the Immediately Supervised Trainee License. This form requires a signature. Electronic signatures will not be accepted.
• Pesticide Apprentice License (APL) application - editable PDF
  This form is required for those applying for a Pesticide Apprentice License. This form requires a signature. Electronic signatures will not be accepted.
• Pesticide Consultant (PC) application - editable PDF
  This form is required for those applying for the Pesticide Consultant License. This form requires a signature. Electronic signatures will not be accepted.
• Pesticide Dealer (PD) application - editable PDF
  This form is required to obtain the Pesticide Dealer License. This license is required to sell restricted-use pesticide products in Oregon.
• Private Pesticide Applicator (PAL) application - editable PDF
  This form is required for those applying for the Private Pesticide Applicator License.
• Public Pesticide Applicator (PPA) application - editable PDF
  This application is required for the Public Pesticide Applicator License. In addition, applicants must take and pass certification exams. This form requires a signature. Electronic signatures will not be accepted.
