You can access our online licensing system with real time licensing data on Oregon optometrists. The system may be used for license verification purposes. If you require a formal, written license verification, please submit a written request and the $40 fee. If you are an optometrist working on licensure in another state and there is a specific form required, please send that to the agency as well.
Any licensee requests for information or to update licensee data via email must use the Board's e-signature. The required elements of an acceptable e-signature are:
Full legal name
License number
Last 4 digits of the licensee's social security number

Ordering Licensee lists
If you would like to order a list of our active or inactive status licensees, click the license search link above and then click "Document Request." That will allow you to do an online order and pay via credit card for the order. Once the payment processes, OBO will email the requested list(s).
The Board performs primary source verification of the highest level of education for each of the licensees.
