Step 2: Download homeowner acknowledgement form

  1. Log in to your account. If you have just created your account, select Login in the upper right corner.

  2. Every online application submitted by a homeowner requires a Homeowner Acknowledgement Form. Select the Homeowners button at the top of the webpage, then click on Homeowner Acknowledgement Form in the list of options.

    This will open the Homeowner Acknowledgement Form in a new tab, which must be filled out and attached to the application. You can fill out the form digitally and save it to your computer, or print it out, fill it, and scan the completed form to your computer. Either way, take note of the file location so that you can upload the form to your application in step 4.
