What Are Home and Community-Based Services?

Home and Community-Based Services (HCBS) are Medicaid-funded services for people living in their home or community instead of a facility, like a hospital. Oregon Health Authority provides these services through a state plan option called 1915(i). These services are available for adults ages 21 and over and help people with long-term mental health challenges live better on their own. 

This page is for people, caregivers, guardians, and families who need support services, as well as for providers. Here, you will find answers to common questions, learn about services and how to access them, find relevant provider information, and more.

The services and supports offered through HCBS are provided to eligible consumers based on their person-centered service plan. The services include:

Community-Based Integrated Supports

To learn social skills and live better in the community.

Transition Services

To move from a facility to a home in the community.

Psychosocial Rehabilitation

To learn or regain skills needed to live and work in their communities.

In-Home Personal Care

Help with bathing, dressing, grocery shopping and taking medicine.

Residential Rehabilitation

Support and training at home to develop skills to live independently.

Home-Based Habilitation

To learn skills like getting around town, handling money, and taking care of themselves.

Housing Support Services

To find a place to live and solve challenges during the process. 

Pest Eradication Services

Getting rid of pests like ants, termites and more. (Not currently available)

Community Transportation

Travel to appointments and community activities. 

Home-Delivered Meals

Delivering meals to people at home. (Not currently available)
