The Public Employee Benefits Board (PEBB) will be hosting flu and COVID-19 vaccination clinics for state employees and their dependents later this year. Getting a flu vaccine is the best way to stop the spread of flu and protect yourself, your family and your coworkers from influenza and related complications. Flu and COVID-19 vaccination are recommended for most everyone who is 6 months and older this fall. Consult with your health care provider if you have any questions. Per the CDC, it is safe to receive flu and COVID-19 vaccinations at the same time.

Please use this Microsoft Bookings form to schedule an appointment. You must fill this out for each individual in your household or family that will be joining you.

- Children 12 and older can be accommodated with parental/guardian consent.
Kids on Medicaid who are VFC-eligible can receive their immunization.
Please request your time at least 48 hours prior to your preferred day. You will receive an automated email with a calendar reminder confirming your appointment. The email you receive will also have a link you can use if you need to cancel or reschedule your appointment.
You may print, fill out and bring the insurance authorization form/s with you to your appointment. Alternatively, the forms will be available for you to fill out when you arrive for your vaccine(s).
Please remember to bring your insurance card to your appointment.
