Ages 6 and older
- Records are not available for patients older than 6 years of age. Newborn bloodspot screening results are kept for 6 years and then are securely destroyed.
- For patients requesting sickle cell test results for college athletics, the laboratory is unable to provide these results.
Younger than 6 years of age
- Medical Providers: Request patient newborn bloodspot screening records by faxing the child’s name, date of birth, and mother’s name and date of birth on your practice letterhead to 503-693-5601.
- Parents or Guardians: Complete the OSPHL Laboratory Test Result Request Form (pdf).
Patients born in other states
Please contact the birth state's newborn bloodspot screening program to request results. Contact information for each state can be found on the
NewSTEPs website.
Online Portal Access for Health Care Providers
The Newborn Bloodspot Screening Program, located at the Oregon State Public Health Laboratory, provides secure, online access to newborn bloodspot screening (NBS) results through the OSPHL Secure Remote Viewer (SRV).

Need to request access?
The applicant must complete and sign the fields on the OSPHL SRV Set-Up Form (pdf). The applicant's form must be authorized by a medical provider in their facility for rights to be granted.
