Nonmedical Vaccine Exemptions

The Oregon Immunization Program serves all people living in Oregon, regardless of their decision to vaccinate. In partnership with Oregon Health and Science University (OHSU), OIP is conducting an optional survey to understand the reason(s) families claim nonmedical exemptions. Thank you in advance for sharing your voice, we value your insight.
There are two ways to claim a nonmedical vaccine exemption in Oregon.

Option 1

Watch an online vaccine education module and submit a certificate of completion.

Parents / Guardians

  1. Watch the Vaccine Education Module for children in child care, preschool, or K-12 and print out the Vaccine Education Certificate at the end of the module.
  2. Fill out the nonmedical exemption section of the Certificate of Immunization Status form (pdf).
  3. Turn in both forms to your child's school or child care.

  1. Watch the College Measles Module.
  2. Print out and sign your Vaccine Education Certificate at the end of the module.
  3. Turn in this form to your college or university.

Option 2

Talk to a health care provider and have them sign a Vaccine Education Certificate.

Parents / Guardians

  1. Meet with a health care provider and get a signed copy of the Vaccine Education Certificate.
  2. Fill out the nonmedical exemption section of the Certificate of Immunization Status form (pdf).
  3. Turn in both forms to your child's school or child care.
