Nonmedical Vaccine Exemptions

The Oregon Immunization Program serves all people living in Oregon, regardless of their decision to vaccinate. In partnership with Oregon Health and Science University (OHSU), OIP is conducting an optional survey to understand the reason(s) families claim nonmedical exemptions. Thank you in advance for sharing your voice, we value your insight.

There are two ways to claim a nonmedical vaccine exemption in Oregon.

Option 1

Watch an online vaccine education module and submit a certificate of completion.

IMPORTANT! Read before you start the module:

To protect your privacy, we do not store any information. For this reason, you will need to start over if you close the module before you are finished.

Allow 15 to 60 minutes to finish this module. The amount of time depends on the number of vaccines for which you are getting an exemption.
If you need to take a break, leave the module window open.
When you reach the end, make sure you print, save, or take a screenshot or picture of your Vaccine Education Certificate before leaving that screen. You will not be able to go back and get your certificate later.

Parents / Guardians

Watch the Vaccine Education Module for children in child care, preschool, or K-12 and print out the Vaccine Education Certificate at the end of the module.
Fill out the nonmedical exemption section of the Certificate of Immunization Status form (pdf).
Turn in both forms to your child's school or child care.

Option 2

Talk to a health care provider and have them sign a Vaccine Education Certificate.

Parents / Guardians

Meet with a health care provider and get a signed copy of the Vaccine Education Certificate.
Fill out the nonmedical exemption section of the Certificate of Immunization Status form (pdf).
Turn in both forms to your child's school or child care.
