Finding the Right Oregon Health Plan (OHP) Provider

Choose Your Primary Care Providers

Your primary care provider (PCP), primary care dentist (PCD) and behavioral health provider and behavioral health provider are important because they:

  • Are your first contacts when you need medical, dental or behavioral health care, unless it's an emergency
  • Manage your medical, dental and behavioral health services and treatments and
  • Arrange your specialty and hospital care.

Your PCP is the doctor, nurse or other provider you want to see first when you have health care needs. (You can also choose a behavioral health provider to be your first contact for care.)

To find a PCP, PCD or behavioral health provider:

​Check the provider directory on your CCO’s website. You can also call your CCO. To find a PCD, call the dental plan listed on your CCO ID card. ​

​Call OHP Care Coordination at 800-562-4620 (Monday through Friday, 8 a.m. to 5 p.m.). 

American Indian and Alaska Native members can call CareOregon Tribal Care Coordination at 844-847-9320​ (Monday through Friday, 8 a.m. to 5 p.m.)​

​Ask your CCO or OHP Care Coordination if you can keep seeing that provider.

If your provider is not a CCO or OHP provider, ask your provider to become an OHP provider.

If your provider is with another CCO in your area, ask your CCO or OHP Client Services about changing your CCO. Learn more about changing CCOs.​

​Ask who they like or don’t like. This can help you choose. ​​

​For example, does the clinic speak your language? You may need someone to translate for you at your health care visit.​

​Call their office and ask:

  • If they will take a new patient
  • If they will accept your coverage (OHP, your CCO and any other coverage you may have)
  • Where they are
  • When they are open
  • How far away they are from you and
  • If they are near public transportation.​


​This is a type of health care clinic that makes you the important part of your care. They keep records about your health, the medicines you take and your needs. They make sure your medical, dental and behavioral care is right for you.

Licensed, Registered or Certified Provider Types

Here is a list of types of Oregon health care professionals.

  • Not all CCOs may cover the services of all these professionals.
  • You may need a referral from your primary care provider (PCP) to see some types of providers. If you do not have a referral, you may have to pay the bill. 
  • No referral is needed for covered mental health, substance use treatment, family planning or related services.

Counselors licensed by the Oregon Board of Licensed Professional Counselors and Therapist who maintain certification as a Gambling Addiction Counselor, Certified by the Mental Health & Addiction Certification Board of Oregon

Dental hygienists

Dental hygienists with an Expanded Practice Dental Hygienist Permit (EPDHP) licensed by the Oregon Board of Dentistry

Dental specialists

Dental Therapist licensed by the Oregon Board of Dentistry

Dentists

Denturists licensed by the Oregon Health Licensing Office

Dietitian, (RDN) and (RD) licensed by the Oregon board of Licensed Dietitians

Direct Entry Midwife licensed by the Oregon Board of Direct Entry Midwifery

Electrologist, licensed by the Oregon State Board of Electrologists and Body Art Practitioners;

Emergency Medical Technician (EMT), Advanced EMT (AEMT), EMT-Intermediate (EMT-I), Emergency Medical Responders (EMR), and Paramedics Licensed by the Oregon Health Authority;
