How the PCPCH Recognition Criteria is developed

In 2009, the Oregon Legislature created the PCPCH Program through passage of House Bill (HB) 2009 as part of a comprehensive statewide strategy for health system transformation. The PCPCH is Oregon's version of the “medical home," which is a model of primary care organization and delivery that is patient-centered, comprehensive, team-based, coordinated, accessible and focused on quality and safety. The PCPCH recognition criteria is developed by OHA in partnership with a Standards Advisory Committee. Members include patients, clinicians, people from health plans, people from community-based organizations and other interested partners. For more information please visit the Standards Advisory Committee webpage.



2025 PCPCH Recognition Standards

Beginning in January of 2025, all practices applying or re-apply for PCPCH recognition do so under the 2025 PCPCH recognition standards and criteria which are described in the PCPCH 2025 Recognition Criteria Technical Specifications and Reporting Guide. The PCPCH 2025 PCPCH Standards Overview and FAQ and the PCPCH 2025 Quick Reference Guide outline the differences between the 2025 standards and the previous 2020 standards. The 2025 Standards Self-Assessment Tool can help practices anticipate what tier level their practice meets under the 2025 standards. 2020 PCPCH Recognition Standards

All practices applying or re-apply for PCPCH recognition between January of 2021 and December of 2024 do so under the 2020 PCPCH recognition standards and criteria which are described in the PCPCH 2020 Recognition Criteria Technical Specifications and Reporting Guide. The PCPCH 2020 TA Guide Revisions Summary and the PCPCH 2020 Quick Reference Guide outline the differences between the 2020 standards and the previous 2017 standards.
