The searchable resource library below contains additional guidance, materials, tools, templates, and other resources that primary care practices might find helpful when engaging with the PCPCH model and standards. Some of these resources come from the PCPCH program direclty, and some were supplied by other health organizations. Use the search tool to find resources related to a specific PCPCH standard or topic (for example if you wanted to find resources related to PCPCH Standard 3.C - Behavioral Health Integration you could type "3.C" or "behavioral health").
