ORS 475A.582 requires the Oregon Health Authority to maintain a telephone hotline for designated city or county employees to verify the address of a location of a premises for which a license has been issued under ORS 475A.210 to 475A.722 or the location of a premises for which an application for licensure has been submitted under ORS 475A.245.
City and county employees must submit an Authorized Personnel Form to localgovverification.psilocybin@oha.oregon.gov before verification may begin.
