Living Well with Chronic Disease Self-Management Programs (CDSMPs)

CDSMPs promote taking control of your life and living healthier

Chronic conditions include diabetes, arthritis, HIV/AIDS, high blood pressure, depression, heart disease, chronic pain, anxiety, and multiple sclerosis. If you or someone you care for has an ongoing health condition, chronic disease self-management programs can help.


For those living with chronic disease
Chronic disease self-management programs help you learn about your health conditions and how to manage your symptoms as well as get the support that you need to take charge of your health. Evidence-based programs include:

  • Living Well With Chronic Conditions and other Self-Management Resource Center Programs such as Chronic Disease Self-Management, Diabetes Self-Management, Chronic Pain Self-Management, and Cancer: Thriving and Surviving
  • Oregon Arthritis Program, Walk With Ease Program
  • A Matter of Balance
  • Tai Chi for Arthritis
  • Falls Prevention for older adults or younger adults at increased risk for falls, Tai Ji Quan: Moving for Better Balance

For those at risk for chronic disease

The National Diabetes Prevention Program (National DPP) is an in-person or virtual evidence-based lifestyle change program developed by the Centers for Disease Control and Prevention (CDC) for preventing type 2 diabetes. Many insurance companies cover the cost of the National DPP and in Oregon Medicaid pays for this program too.

The Oregon Self-Management Network

Network Vision

To support the organizations that affect change within our communities through expansion of sustainable evidence based self-management programs that support better health, better care and lower costs.

Network Purpose

  • Promote successful statewide expansion, implementation, and coordination of evidence-based chronic disease self-management programs
  • Facilitate communication and collaboration between organizations that support self-management programs in Oregon
  • Provide a venue for addressing challenges and opportunities that affect all programs and partners
  • Support the sustainability of programs statewide by enhancing local ownership of programs and sharing knowledge and resources

Membership

The Network is open to all organizations providing evidence-based self-management programs in Oregon and to other partners interested in self-management, including representatives from health systems, providers, insurers, and community-based organizations. We also encourage participation from self-management program leaders.

NEW: The Healthy Heart Ambassador Program

The Healthy Heart Ambassador Blood Pressure Self-Monitoring Program (HHA-BPSM) is a 4-month duration evidence-based program that helps participants to lower and manage their blood pressure. The program empowers participants to measure and monitor their blood pressure using proper technique, provides individualized support from a program facilitator and nutrition education for participants to learn about ways to eat healthier. The program can be held at a variety of locations; each site should have space for office hours and for the nutrition education seminars. The program can also be virtual.

Healthy Heart Ambassadors (HHAs) are trained program facilitators who support participants to track and manage their blood pressure and learn about healthy eating. HHAs guide and encourage participants to find their own path to better blood pressure management, they do not diagnose or treat high blood pressure.

HHA-BPSM was developed by the Y-USA for implementation in YMCA Associations. HHA-BPSM has been offered by Y-USA and the Centers for Disease Control and Prevention (CDC) for use in non-YMCA community-based settings.

Oregon Health Authority Health Promotion and Chronic Disease Prevention (HPCDP) staff members are available to train program facilitators in our state. If you are interested to offer this program to your community, please email living.well@dhsoha.state.or.us for details.
