This page lists resources for organizations awarded behavioral health housing and residential treatment funding through the Behavioral Health Investments Regional Innovation and Investment Initiative.

​​​Deadlines and Miles​​​​tone Deliverables

Consolidated quarterly reports​ are due to OHA no later than:

  • April 15 (January 1-March 31), 
  • July 15 (April 1- June 30), 
  • October 15 (July 1- September 30), and 
  • January 15 (October 1-December 31). 

​​​Recipient shall submit to OHA their Annual Report using the template provided by OHA.

Recipient shall submit a Project Performance Plan to OHA within 45 days of the effective date of this Agreement.

  • Execute their Grant Agreement
  • Sign the Declaration of Restrictive Covenants
  • Submit Certificate of Insurance
