The Patient-Centered Primary Care Home (PCPCH) Program is part of Oregon's efforts to fulfill a vision of better health, better care and lower costs for all. The program collaborates with community partners to develop standards of care for primary care practices and then certifies practices meeting those standards as a recognized PCPCH.
