CCO Form A Homepage

OHA encourages all entities to request a conference prior to filing a CCO Form A application. During a pre-filing conference, OHA and entities can discuss the filing process, whether the proposed transaction requires entities to file an application, what forms will need to be completed, what information may be requested from entities, handling of confidential information, and submission timing. To request a pre-filing conference, please email CCO.FormA@oha.oregon.gov

If the proposed transaction is also subject to Health Care Market Oversight (HCMO) program review, entities can request a joint pre-filing conference for both review processes. Entities can request the joint pre-filing conference through the CCO Form A email or the HCMO email
