Conflict of Interest

These conflict of interest forms were created for transparency about the perspectives of members of the HERC and its subcommittees as well as appointed ad hoc experts for specific topics.

To apply to serve as a subcommittee or expert, please use the appropriate form for your potential role and inform us of your interest by emailing us at herc.info@dhsoha.state.or.us. Include a copy of your curriculum vita. Those interested in service on the Health Evidence Review Commission can apply through executive appointments at the Governor’s office.
