Licensing process and requirements

New construction
Before applying for a building permit, prospective applicants who intend to build and license a new facility as a residential care or assisted living facility must comply with the process as outlined in OAR 411-054-0012.

Initial licensure / change of ownership
Applicants for residential care and assisted living facilities must provide ODHS with a complete licensing application (SE 0570) at least 60 days before expected licensure.
