How to Renew your License:
- Renewal each year is November 1—December 31 (850-030-0195)
- To renew log into the
Self-Service Portal click on "License Renewal", report 32 annual CE (850-040-0210), answer the declaration questions/personal history, complete the OHA survey and pay renewal fees.
- Renewals must be submitted on or before Dec. 31 by 11:59pm. Payment by debit card or credit card.

Pain Management Requirement
One credit hour of pain management every odd number calendar year. Pain management credit(s) earned may be applied to annual continuing education required for license renewal. Click here for the pain management rule.
Cultural Competency Requirement
One credit hour of cultural competency every even number calendar year. Cultural competency credit(s) earned may be applied to annual continuing education required for license renewal. Click here for the cultural competency rule.
Annual Renewal Fees:
Active License - $437 includes the PDMP fee and OHA survey fee
Inactive - $192 includes the OHA survey fee
Retired - $15
Natural Child Birth - $60
