The Board
of Naturopathic Medicine maintains an online license verification system of
licensed naturopathic doctors in Oregon.
Each
licensee record will note if there is any Board Disciplinary Action. Board disciplinary action including but not
limited to Notices, Final Orders and Settlement Agreements. State law prohibits the disclosure of
complaint information.

Licensee Change of Address:
OAR 850-050-0130 require all licensees to maintain a current home, mailing and work address with corresponding telephone numbers and email address on file with the Board at all times. Any change in name, address, or contact information noted above must be reported to the Board within 30 days following the event. To change your contact information log into the
License Portal.

License Verification:
To request a License Verification, the licensee will need to log in to their license portal then navigate to the Document Request form. On that form, the licensee will need to provide the mailing address of the organization that they want the verification sent to. Please ensure that the address provided is correct and complete.
